The following forms are required for St. Francis' Youth Events:
(Please print, fill out and turn in at the Youth Office)
Form A - this is the Medical Release/Liability Form that is good from June 1, 2008 through May 31, 2009. Only needs to be filled out 1 time per year.
Form B - this is the Event specific form that is required when we go away from the church and needed each time.
Form D - this is the Adult Medical Release/Liability Form that we keep on file for the year like Form A.
